Meaning of the commentary

After discussing faults of insertion, the commentary turns to defects of the nozzle. ‘As before’ first refers to care like that for a fresh wound. A collar placed too near makes the administration futile; avaseka means leakage. The commentary describes a collar placed beyond the stated position and contrasts a blunt-ended instrument with the sharper basti nozzle, which it says may press a vital anal site and therefore calls for the indicated care. A short nozzle is one below the appropriate measure, and a narrow channel has a very small opening; delay caused by forcing material through it is called difficulty. Returned material may, according to this explanation, lead to basti-obstruction disorders such as retention or painful passage of urine, for which the corresponding treatment is implied rather than stated. Jejjaṭa gives an alternative interpretation: ‘basti as before’ means a picchā basti. For a nozzle that is long and wide, the commentary compares the result to excessive pressure, including discharge through the nose or mouth, and refers to care for pressure on the throat and related complications.

If the basti bag is too broad or too thick, the defect is said to resemble one that is poorly tied. If the bag is too small, the quantity of material is correspondingly small and its attributed benefits are considered limited. Meaning of the commentary

Having described defects of the nozzle and their responses, the commentary introduces defects of the basti bag. ‘Too small’ means below the appropriate size; because the bag holds less, the medicinal material is also reduced, and therefore the source attributes fewer benefits to it. ‘Like a broken nozzle’ means that just as material leaks through a broken nozzle, it also leaks from a poorly tied or torn bag.

Meaning of the śloka

If the basti reservoir is compressed too forcefully, the material is said to pass toward the stomach and, driven by vāyu, emerge through the nose or mouth. The text describes immediate countermeasures. If compression is too weak, the material does not reach its intended location or accomplish its purpose. Repeated compression is said to confine vāyu within the abdomen, producing distension and severe pain; the corresponding basti is prescribed for each condition. Delay beyond the proper time causes distress and aggravates the disorder, for which another basti directed against the strength of that disorder is advised.

Meaning of the commentary

After discussing defects of the basti apparatus, the commentary turns to faults of compression. It glosses prapadyate as 'comes out' and avadhūnana as lifting and shaking by the hair or similar means. The cold affusions named by the commentator include milk, sugarcane juice, a decoction of yaṣṭimadhu, fermented grain liquid, cukra, and other cooling substances. Slowly compressed means weakly compressed; such administration does not perform its intended function. 'Within' is supplied as 'within the abdomen,' and the resulting internal vāyu is said to cause distension and pain. Yathāsvam bastayaḥ means the basti considered appropriate as a countermeasure for each respective condition. Regarding delay, the commentator explains that prolonged retention of the nozzle obstructs the anal passage, hinders vāyu that is ready to move, and thereby aggravates a disorder understood to improve through anulomana.

Meaning of the śloka

Unprocessed oil is said to cause coating and swelling of the rectum; the text recommends a cleansing basti and purgation. An insufficient amount of either kind of basti has little effect, whereas an excessive amount is said to cause obstruction, exhaustion, and diarrhea. Excessively hot or sharp preparations are associated with fainting, burning, diarrhea, and an increase of pitta; overly mild or cold preparations are associated with obstruction of vāyu and abdominal distension. It preserves a reading attributed to Gayadāsa: where the received verse has 'purgation is also beneficial,' he reads 'dry purgation is also beneficial.' Meaning of the śloka

The text next describes complications attributed to unsuitable patient positioning. With the head too low, effects like those of excessive compression are said to occur; with the head raised, basti is said to produce distension and trouble involving the urinary or genital region. It then names a historical remedy. In a face-down position, the material is said to take a wrong path and fail to reach the pakvāśaya, while vāyu troubles the heart region, rectum, and abdomen. Lying face-up is said to obstruct the route so that basti does not enter, and movement of the nozzle is said to disturb vāyu internally. When the body or both thighs are contracted, the basti is said not to return properly. Given while standing, it returns quickly downward and fails to nourish its intended site. In a right-side position, it is said not to reach the whole pakvāśaya because that structure is described as following the left side. The text therefore disapproves administration in these positions and says that subsequent aggravation of vāyu should be addressed according to the affected condition.

Meaning of the commentary

After material faults, the commentary introduces faults connected with the bed or the patient's position. For the raised-head case, it says Gayī understood the text as recommending uttara-basti after adequate fomentation. Jejjaṭācārya preserves a different reading and explains the phrase about oily urination; for him, the countermeasure is a cleansing nirūha for a well-uncted patient, explicitly not uttara-basti. Vṛddha-Suśruta preserves another wording, describing distension associated with basti and difficult urination. The commentary explains that in the face-down position the material follows a wrong route and does not reach the pakvāśaya. Netrasaṃvejana means shaking or movement of the nozzle, said to disturb vāyu. With contracted thighs the material does not return or come out; avāṅmukha means facing downward. For the right-side position, the stated reason is that the pakvāśaya follows the left side. The closing advice is interpreted as avoiding the causal positions and treating trouble of the heart region, back, rectum, or other site with a basti prepared with a medicine appropriate to the respective disorder.

Meaning of the śloka

The text describes complications beginning with inadequate action (ayoga) and their remedies. A basti that is insufficiently warm, contains too little medicine, or is too small may fail to return as expected; obstruction, abdominal distension, and pain are called ayoga. It then attributes marked distension and severe pain in the cardiac region, waist, flanks, and back to factors such as residual or recently eaten food, abundant doṣa, excessive quantity, insufficient warmth, or unsuitable unctuousness and salt. An excessively sharp, salty, or dry basti is said to aggravate vāyu together with pitta and cause cutting pain around the navel, bladder region, and anus. Excessive sourness, salt, or sharpness is linked with discharge, weakness, exhaustion, burning, and difficult evacuation of mucus and blood. Other described complications include distress affecting the heart, bodily pain, intoxication-like disturbance, fainting, heaviness, stiffness, lassitude, yawning, twisting, tremor, and joint pain. Excessive heat, sharpness, quantity, or use after excessive sweating or when little doṣa is present is said to produce atiyoga, whose signs resemble excessive purgation; a still more severe state includes loss of blood. The nine complications stated for nirūha are also to be recognized in sneha-basti. The physician is enjoined to act so that these complications do not arise.

Meaning of the commentary

The commentary distinguishes too little medicine from too small an overall quantity: the former implies weak potency through lack of sharpness, while the latter concerns the total measure. Failure to return is recognized through obstruction and related signs. Its explanation of the corrective basti is broader than the single word 'sharp': it is described as reversing the deficient qualities by being warm, sufficiently medicated, and of appropriate measure. It differentiates nirūha given while food remains undigested or after eating from anuvāsana, for which administration with food is regarded differently in the source. It also separates the discharge described here, taken as predominantly pitta-related, from the kapha-pitta discharge associated with excessive emesis or purgation. The commentary preserves conditional distinctions among pitta-rakta predominance, vāta-rakta predominance, and association with kapha, and it expands the varieties of pain to include piercing, burning, and itching sensations. For the passage about a cleansing basti, it names a classical compound containing ingredients including gomūtra and trivṛt and explains the intended rationale as drawing downward doṣa that has moved upward and pacifying disturbed doṣas. Here 'dry' is also glossed as a person of vāta constitution. 'Cooling' is extended to cooling care generally. Meaning of the commentary

The commentary presents this as guidance about the order and intervals of the five procedures. It argues that after properly performed vamana, virecana should occur at a fortnight: doing it earlier is said to risk complications because digestive and bodily strength remain weak, while waiting too long is said to interrupt the effects of prior oleation and sweating. It then lays out a detailed historical sequence involving dietary restoration, oleation, sweating, virecana, later anuvāsana according to doṣa, and subsequent āsthāpana. A distinct interpretation attributed to Gayadāsa, based on a statement of Caraka, allows either drinking ghee or anuvāsana according to the person's condition after dietary restoration. The commentary further says Gayadāsa described different circumstances for āsthāpana preceded by internal oleation or by anuvāsana, including incomplete purgation with residual material and obstructed vāyu, or particular disease states. After nirūha, anuvāsana is interpreted as occurring that same day after the prescribed meal; after virecana it is delayed until strength and food intake have returned. The commentator notes that some give a different explanation, but rejects it because Gayadāsa had criticized it in several ways. The alternatives and disagreement are preserved here without treating any schedule as current clinical guidance.
