Twelve are treated without instruments, and seven are manageable rather than fully curable. A knowledgeable physician should avoid treating fifteen disorders. Meaning of the commentary

Of the eight classical kinds of operative work, four are applied to eye disorders. The passage therefore groups, in summary, the diseases considered eligible for those four kinds of instrument-based treatment. The commentary adds that the word ‘and’ after the twelve non-instrument cases also includes the two externally caused disorders. ‘Manageable’ means maintained through measures beginning with snehana, rather than through excision or the other listed operations. For the two external disorders, the classification as manageable or incurable depends on their state. Disorders to be avoided are explained as incurable.

The commentary identifies the four pavana disorders as Hatādhimantha, Nimiṣa, Gambhīradṛṣṭi, and Vātahatavartman. The two pittaja disorders are Hrasvajāḍya—with the variant reading Hrasvajātya—and Jaladhisrāva; the single kapha-caused disorder is Kaphasrāva. It explains aṣṭārdhaka as ‘four’ and names the four blood-origin disorders as Raktasrāva, Ajakājāta, Śoṇitārśas, and Vraṇānvitaśukra.
