Overview

A fistula-in-ano is an abnormal tunnel that connects the anal canal or rectum to the skin around the anus. It most commonly develops following an infection of the anal glands that leads to an abscess. Even after the abscess drains or is treated, a persistent tract may remain, resulting in recurrent discharge, pain and repeated infections.

Anal fistulas rarely heal on their own and often require a definitive procedure to eliminate the tract while preserving normal anal sphincter function.

At AYANA, every fistula is carefully evaluated to determine its complexity, relationship to the anal sphincter and associated branches. Treatment is individualized and may include Kshara Sutra Therapy, surgery or a combination of procedures, with the goal of complete healing, preservation of continence and minimizing recurrence.

Causes & Risk Factors

Most anal fistulas develop after an infection of the anal glands.

  • Previous perianal abscess
  • Chronic anal gland infection
  • Crohn’s disease
  • Tuberculosis (selected cases)
  • Previous anorectal surgery
  • Radiation therapy
  • Trauma
  • Poorly controlled diabetes
  • Reduced immunity

Symptoms

Common symptoms include:

  • Persistent or recurrent discharge near the anus
  • Recurrent swelling around the anus
  • Pain, especially while sitting or during bowel movements
  • Recurrent perianal abscesses
  • Blood-stained or pus-like discharge
  • Skin irritation around the external opening
  • Foul-smelling discharge
  • Fever during acute infection

When Should You See a Doctor?

Medical evaluation is recommended if you experience:

  • Persistent discharge near the anus
  • Recurrent painful swelling
  • Repeated perianal abscesses
  • A non-healing opening near the anus
  • Fever associated with anal pain or swelling
  • Symptoms that persist despite previous treatment

Early diagnosis helps prevent progression into more complex fistulas and recurrent infections.

Diagnosis

A thorough evaluation is essential because treatment depends on the type and complexity of the fistula.

  • Clinical examination
  • Identification of the external opening
  • Digital rectal examination
  • Proctoscopy
  • MRI Fistulogram (for complex or recurrent fistulas)
  • Examination under anaesthesia when required

Fistulas are classified according to their relationship with the anal sphincter, which guides treatment planning.

Treatment Options at AYANA

Treatment is individualized based on the anatomy of the fistula, its complexity, previous surgeries and overall health.

Infection Control

If an active abscess is present, drainage may be required before definitive treatment. Associated infection and inflammation are managed before proceeding with fistula treatment.

Ayurvedic Medicines

Selected Ayurvedic formulations may be prescribed to:

  • Support wound healing
  • Reduce inflammation
  • Promote healthy bowel habits
  • Improve overall recovery

Kshara Sutra Therapy

Kshara Sutra Therapy is one of the most established Ayurvedic para-surgical treatments for anal fistula. A specially prepared medicated thread is placed within the fistulous tract, where it facilitates controlled tract division, drainage, debridement and healing from within while preserving surrounding healthy tissue.

Weekly thread changes are performed until complete healing is achieved.

Surgical Management

Depending on the anatomy of the fistula, surgical procedures may be recommended either alone or in combination with Kshara Sutra Therapy. The choice of treatment is individualized after detailed clinical assessment.

Wound Care & Follow-up

  • Regular wound assessment
  • Dressing changes when required
  • Weekly Kshara Sutra changes (where applicable)
  • Bowel regulation
  • Dietary advice
  • Scheduled follow-up

Recovery & Aftercare

Healing depends on:

  • Length of the fistulous tract
  • Complexity
  • Number of branches
  • Presence of infection
  • Previous surgeries
  • Patient compliance

Patients are advised regarding local hygiene, sitz baths when appropriate, diet, hydration, prevention of constipation and regular follow-up until complete healing.

Prevention

Although not all fistulas can be prevented, the following measures may reduce the risk:

  • Early treatment of perianal abscesses
  • Good perianal hygiene
  • Prompt treatment of anorectal infections
  • Good diabetes control
  • Healthy bowel habits
  • Early medical evaluation for recurrent anal symptoms

Frequently Asked Questions

Can a fistula heal without treatment?

Anal fistulas rarely heal spontaneously because the tract usually remains open. Definitive treatment is generally required.

Is every fistula treated with Kshara Sutra?

No. While Kshara Sutra Therapy is highly effective for many fistulas, the most appropriate treatment depends on the fistula’s anatomy, complexity and relationship to the anal sphincter.

Is surgery always necessary?

Most fistulas require a definitive procedure, which may include Kshara Sutra Therapy, surgery or a combination of techniques. The treatment plan is individualized.

Can a fistula recur?

Yes. Recurrence can occur with any treatment, particularly in complex fistulas. Careful pre-operative assessment, appropriate procedure selection and regular follow-up help reduce this risk.

Will treatment affect bowel control?

Preserving anal sphincter function is an important consideration in fistula management. Treatment is planned to achieve healing while minimizing the risk of continence problems whenever possible.

How long does treatment take?

Healing varies depending on the complexity of the fistula. Simple fistulas may heal within weeks, whereas complex fistulas may require a longer treatment course with regular follow-up.
