Overview

Hemorrhoids, commonly known as piles, are enlarged and symptomatic vascular cushions within or around the anal canal. While these vascular cushions are a normal part of human anatomy and contribute to continence, they can become enlarged, bleed, prolapse or cause pain under certain circumstances.

Common contributing factors include chronic constipation, prolonged straining during bowel movements, prolonged sitting on the toilet, pregnancy, obesity and low dietary fibre intake.

At AYANA, treatment is individualized according to the type and severity of hemorrhoids, symptoms, overall health and patient preferences. Depending on the condition, management may range from dietary and lifestyle modification to Ayurvedic medicines, Kshara Karma or surgical intervention when required.

Causes & Risk Factors

Several factors increase the likelihood of developing symptomatic hemorrhoids, including:

  • Chronic constipation
  • Repeated straining during defecation
  • Hard stools
  • Prolonged sitting on the toilet
  • Pregnancy
  • Obesity
  • Low-fibre diet
  • Sedentary lifestyle
  • Advancing age
  • Chronic cough
  • Heavy lifting
  • Family predisposition

Symptoms

Symptoms vary depending on the type and stage of hemorrhoids and may include:

  • Bleeding during or after bowel movements
  • Bright red blood on toilet paper or in the toilet bowl
  • Anal swelling
  • Prolapse (a swelling that protrudes during defecation)
  • Pain (especially in thrombosed external hemorrhoids)
  • Itching around the anus
  • Mucous discharge
  • Difficulty maintaining local hygiene
  • Sensation of incomplete evacuation
  • Perianal discomfort

When Should You See a Doctor?

Seek medical evaluation if you experience:

  • Recurrent rectal bleeding
  • Persistent pain or swelling
  • A prolapsing anal mass
  • Sudden painful swelling around the anus
  • Bleeding associated with weight loss or altered bowel habits
  • Symptoms that do not improve with conservative care

Not all rectal bleeding is due to hemorrhoids. A proper examination is essential to exclude other anorectal or colorectal conditions.

Diagnosis

Diagnosis begins with a detailed history and clinical examination.

  • Inspection of the anal region
  • Digital rectal examination
  • Proctoscopy
  • Anoscopy
  • Sigmoidoscopy or colonoscopy when indicated, particularly in patients with atypical symptoms or risk factors

Hemorrhoids are commonly classified into four grades based on the degree of prolapse, which helps guide treatment planning.

Treatment Options at AYANA

Treatment is tailored to the severity of the disease, associated symptoms and individual patient factors.

Lifestyle & Dietary Measures

  • High-fibre diet
  • Adequate hydration
  • Avoiding prolonged straining
  • Healthy bowel habits
  • Regular physical activity
  • Weight management where appropriate

Ayurvedic Medicines

Selected formulations may be prescribed to:

  • Improve bowel regularity
  • Reduce inflammation
  • Control bleeding
  • Support vascular health
  • Promote healing

Kshara Karma

Selected internal hemorrhoids may be managed using physician-controlled Kshara Karma, a classical Ayurvedic para-surgical procedure that produces controlled chemical cauterisation of hemorrhoidal tissue.

Surgical Management

For advanced, prolapsed or complicated hemorrhoids, surgical procedures may be recommended. The choice depends on the grade of hemorrhoids, symptom severity and patient factors.

Management of Thrombosed External Hemorrhoids

Patients presenting with an acutely painful thrombosed external hemorrhoid may benefit from early surgical evacuation or excision in appropriately selected cases.

Post-treatment Care

  • Dietary counselling
  • Bowel regulation
  • Pain management
  • Sitz baths when indicated
  • Local wound care
  • Scheduled follow-up

Recovery & Aftercare

Recovery depends on the treatment performed. Most day-care procedures allow patients to return home on the same day.

  • Diet
  • Hydration
  • Physical activity
  • Toilet habits
  • Wound care (if applicable)
  • Follow-up visits

Maintaining healthy bowel habits is essential to reduce the risk of recurrence.

Prevention

Simple lifestyle measures can help reduce the risk of developing hemorrhoids or prevent recurrence:

  • Eat a fibre-rich diet
  • Drink adequate water
  • Avoid prolonged straining
  • Respond promptly to the urge to pass stools
  • Exercise regularly
  • Maintain a healthy body weight
  • Limit prolonged sitting on the toilet

Frequently Asked Questions

Are hemorrhoids and piles the same?

Yes. "Piles" is the common term for hemorrhoids.

Do all hemorrhoids require surgery?

No. Many patients improve with dietary changes, lifestyle modification, medicines or minimally invasive procedures. Surgery is reserved for selected cases.

Can hemorrhoids come back after treatment?

Recurrence is possible if contributing factors such as constipation and straining persist. Long-term bowel care is important regardless of the treatment performed.

Is bleeding always caused by hemorrhoids?

No. Rectal bleeding can result from several conditions, including fissures, inflammatory bowel disease, colorectal polyps and colorectal cancer. Medical evaluation is important, particularly if bleeding is recurrent or associated with other symptoms.

Are Ayurvedic treatments painful?

Most conservative treatments are well tolerated. Para-surgical or surgical procedures are performed under appropriate anaesthesia or pain control measures when indicated.

How long does recovery take?

Recovery varies with the treatment. Conservative management has no downtime, while day-care procedures and surgery may require a short recovery period. Your physician will provide individualized guidance.
