Meaning of the śloka

Now the treatment of vidradhi will be explained, as taught by Bhagavān Dhanvantari. Six kinds of vidradhi have been described. Among them, the type arising from all the doṣas together is incurable. In the remaining kinds, while the swelling is still unripe, treatment like that prescribed for śopha should be undertaken promptly.

Meaning of the commentary

The commentator explains that the mention of six excludes vidradhi produced by a combination of two doṣas. Its incurability is stated again because treatment is now beginning, in order to prohibit intervention in that incurable type. For the other five types, treatment like that for śopha means the eleven measures beginning with apatarpaṇa and ending with virecana.

Meaning of the śloka

For vāta-type vidradhi, the text prescribes a thick, comfortably warm application prepared from pastes of vāta-pacifying roots with ghee, oil, and animal fat. A nourishing poultice is described using meat from marsh-dwelling and aquatic animals, the Kākolyādi group, fats, sour substances, and salt. Continuous fomentation is advised with prepared meat, kṛśarā, milk, and pāyasa, together with removal of blood. If, despite these measures, the lesion advances toward suppuration, it is to be brought to maturity, opened with an instrument, and cleansed. After washing with a Pañcamūla decoction, it is to be filled with salt-predominant oils combined with the Bhadrādi group and Madhuka. Cleansing is then described with a Trivṛt-based preparation combined with purgative substances, followed by healing treatment with a Trivṛt preparation processed with the Pṛthakparṇyādi group.

Meaning of the commentary

The commentary first explains the ingredients and preserves variant readings. Vāta-pacifying substances may mean the Bhadradārvādi group; another interpretation takes vātāghna as Śobhāñjanaka. Some read ‘paste of Muraṅgī root,’ and Muraṅgī is also identified as Śobhāñjanaka. The marsh-animal preparation is characterized as a kind of sālvana: marsh animals include buffalo and the aquatic class includes shell-dwelling creatures; tarpaṇa substances are nourishing and include the Vidārigandhādi group said to pacify vāta and pitta, along with fats and sour substances. The salt is salt processed with these ingredients. Veśavāra is a particular prepared meat dish; kṛśarā is a gruel made from sesame, rice, and māṣa; pāyasa is rice cooked in milk. ‘Salt-predominant oils’ means oils in which salt is foremost, and Bhadrādi refers to Bhadradārvādi. For the cleansing preparation, one explanation says Trivṛt-based fat mixed with powdered Trivṛt and other purgative substances, while Gayī explains it as fat cooked with purgative substances. The Pṛthakparṇyādi oils and ghees are those stated in the mixed healing-ghee formulation.

Meaning of the śloka

For pitta-type vidradhi, the passage describes applications made with Śarkarā, parched grain, Madhuka, and Sārivā, or with Payasyā, Uśīra, and Candana ground in milk. It also describes irrigation with boiled or cold infusions, milk, sugarcane juice, or Jīvanīya ghee combined with sugar. Powdered Trivṛt and Harītakī mixed with honey is prescribed, and blood is to be removed with leeches. When the lesion has suppurated, the knowledgeable practitioner is to open it. It is then washed with a decoction of latex-bearing trees or aquatic plants, covered with a thin application of sesame and Yaṣṭīmadhuka mixed with honey and ghee, and bandaged with a fine cloth. Ghee processed with the listed cooling and wound-healing substances, milk, Kṣīraśuklā, Pṛthakparṇī, Samaṅgā, Rodhra, and Candana, or with shoots or bark of the Nyagrodhādi group, is described for healing the wound.

Meaning of the śloka

The text lists young leaves and fruits of Naktamāla; leaves of Sumanā, Paṭola, and Ariṣṭa; the two Haridrās; beeswax; Madhuka; Tiktarohiṇī; Priyaṅgu; Kuśa root; bark of Nicula; Mañjiṣṭhā, Candana, Uśīra, Utpala, the two Sārivās, and Trivṛt. Equal karṣa portions of these are to be processed with a prastha of ghee. The resulting Karañjādi ghee is praised for calming duṣṭavraṇa, cleansing nāḍīvraṇa, and treating freshly incised wounds. The passage further states that neglected wounds, sinus-like and deep wounds, recent incisions, and even severe wounds caused by fire or alkali subside through this preparation.

The discussion then turns to vidradhi caused by blood and by an external factor. The wording that relates the two following groups, Varuṇādi and Ūṣakādi, contains an uncertainty marker in the supplied commentary.
