Now we shall explain the diagnosis of mūḍhagarbha, an obstructed or abnormally positioned fetus. Thus spoke the revered Dhanvantari.

Meaning of the commentary

The commentary explains mūḍha as ‘one whose movement is obstructed.’ It cites a definition: a fetus whose limbs are fully formed and who possesses mind, intellect, and related faculties is called mūḍhagarbha when confused or obstructed by deranged apāna.

Meaning of the śloka

The passage lists numerous circumstances said to loosen a fetus from its attachments: sexual intercourse, travel in vehicles or on animals, walking a long road, stumbling, falling, severe pressure, running, injury, uneven posture in lying or sitting, fasting, suppression of natural urges, excessively dry, pungent, or bitter food, grief, excessive use of alkali, diarrhoea, vomiting, purgation, swinging, indigestion, measures intended to cause pregnancy loss, and similar factors. Like an unripe fruit separated from its stalk by force, the fetus is described as becoming detached, passing beyond the uterus, descending through spaces near the liver, spleen, and intestines, and disturbing the abdomen. The resulting disturbance is said to derange apāna, producing one or more of pain in the flanks, bladder region, head, abdomen, or vagina, distension, or urinary retention, and to expel an immature fetus with bleeding. If that same fetus has grown, enters the birth passage incorrectly, fails to emerge, and is rendered unresponsive by deranged apāna, it is called mūḍhagarbha.

Meaning of the commentary

The commentary explains the listed causes individually: grāmyadharma means sexual intercourse; yāna and vāhana refer respectively to conveyances such as a chariot and mounts such as a horse; adhvagamana is travel on foot; praskhalana is stumbling; prapatana is a severe fall, such as from a building; prapīḍana and pradhāvana are excessive pressure and running; abhighāta is a blow from a weapon, clod, or similar object; vegābhighāta is suppression of urges such as flatus; preṅkholana is swinging; and garbhaśātana is causing pregnancy loss with medicine. It notes that some read this prose differently but does not record that reading because it considers it inauthentic. Here, ‘attachments’ means the vessels and muscular tissues connected with the uterus. The fruit simile indicates premature loss caused by the stated factors, as an unripe fruit falls through trauma. The commentary glosses the fetus as crossing or passing beyond its place and descending through intervening spaces; koṣṭha includes locations such as the stomach and large intestine, and saṅkṣobha means disturbance or movement. Mūḍha apāna is described as moving in an improper course. Taruṇa denotes an immature fetus whose limbs are not firm and whose consciousness is not manifest. For the derivation of mūḍhagarbha, ‘incorrectly entered’ means coming with a reversed part rather than descending normally; apatyapatha is the vaginal birth passage, ‘not expelled’ means not emerging, and sammohita is glossed as unconscious or fainted.

Meaning of the śloka

Some authorities describe four forms of mūḍhagarbha: kīla, pratikhura, bījaka, and parigha. Kīla is the presentation in which raised arms, head, and feet obstruct the vaginal opening like a peg. Pratikhura is the state in which hands, feet, and head have emerged while the trunk remains caught. Bījaka is the presentation in which the head and one arm emerge together. Parigha is the transverse position that blocks the vaginal opening like a door bar. The text rejects this as a complete fourfold classification, because a fetus affected by disturbed vāyu may enter the birth passage in many different ways, making any fixed number insufficient.

Meaning of the commentary

The commentary says these forms follow from incorrect presentation. It glosses kāyasaṅgī as a body or trunk that is obstructed. In the description of bījaka, ekaśirobhuja means that one arm emerges together with the head. The comparison in parigha is to a bar or bolt used to fasten a door.

Meaning of the śloka

The passage then summarizes eight possible courses of mūḍhagarbha: both legs may enter the vaginal opening; one bent leg may enter; the fetus may lie crosswise with the legs and body flexed and the buttock region presenting; the chest, side, or back may block the vaginal opening; the head may be turned inward toward the side while one arm enters; the head may be bent while both arms enter; the middle of the body may be bent while hands, feet, and head present; or one leg may enter the vaginal opening while the other extends toward the anus. These are stated to be eight forms in summary.

Meaning of the commentary

The commentary explains ābhugna as bent or contracted and repeatedly supplies the understood phrase ‘enters the vaginal opening’ where the terse source omits it. A body with bent legs is likewise understood as entering that opening. Pidhāya means covering or blocking. Antaḥpārśvāpavṛttaśirāḥ is explained as a head placed or turned into the region between the sides. The same omitted phrase about entering the vaginal opening also applies to the cases involving one arm, two arms, and a bent middle. The list is eightfold only as a concise summary; in fuller detail, variations involving transverse neck, thigh, and lower-leg positions are innumerable.

Meaning of the śloka

Among the forms of mudhagarbha described, the last two are said to be incurable. The text also advises leaving aside the remaining forms when the mother is afflicted by disturbed perception, convulsions, displacement or constriction of the birth passage, makkalla pain, breathlessness, cough, or giddiness.

Meaning of the commentary

The commentary identifies the last two forms as the seventh and eighth and calls them incurable by their inherent nature. It then explains that other forms may also become incurable because of the mother's condition. Viparitendriyartha means erroneous apprehension of objects such as visible forms; yonibhramsha is outward displacement of the yoni, and yonisamvarana is constriction of it. Makkalla is described as pain after delivery in the bladder region and related sites when vayu is obstructed by blood. Bhrama is a sensation of turning like that experienced on a wheel.

Meaning of the śloka

As a fruit, when matured by time, naturally separates from its stalk and falls, and otherwise certainly does not fall in that natural way, so a fetus situated in the uterus, when the proper time has advanced and it is released from its bindings to the channels, proceeds toward birth.

Meaning of the commentary

The commentary uses these two verses to explain why mudhagarbha does not occur in natural delivery at the proper time. A fruit reaches ripeness through the passage of time, separates from its stalk, and then naturally falls; without that ripening it does not fall in this manner. Likewise, after sufficient time, the fetus in the uterus is released from its connection with the nadis and proceeds to birth.

Up to the fourth month, loss of the developing fetus is described as occurring in the form of discharge. Thereafter, in the fifth and sixth months, loss of one whose body has become firm is described as a fall or expulsion.

Meaning of the commentary

The commentary glosses garbhavichyuti as release or loss of the fetus and prasravet as taking the form of discharge. On this basis it says that mudhagarbha is not recognized during the first four months. After the fourth month, when the fetus is considered to have solid bodily parts, the loss is described as pata rather than srava; the commentator therefore relates mudhagarbha to the period after the fourth month.

The commentary presents these as signs that the obstructed-fetus condition in the pregnant woman is considered incurable in the classical account. It explains pravidhyati as striking or shaking the head forcefully. Other readers instead have apaviddhashira, understood as having the head bent downward. Nirapatrapa is glossed as being without shame or restraint, and niloddhatasira as having dark, raised veins. The final statement is explained to mean that the woman endangers the fetus and the fetus, in turn, endangers the pregnant woman in that manner.

Meaning of the commentary

The commentary introduces this as extraction of a fetus that remains alive after the mother's death. It explains bastamara through a comparison with the rapid killing of a goat by stopping its breathing or twisting its neck, indicating a sudden death; this comparison is historical and disturbing. Tatkshanat is interpreted as within two ghatikas because the fetus is believed also to die after that interval. Another reading has muhurta, which the commentator says conveys the same point. Janmakala means the time around the ninth month or another stage suitable for birth, and patayitva means cutting open the woman's abdomen. The commentary then quotes a longer passage that some transmit as signs of a fetus dead in the womb, including marked abdominal swelling and severe systemic distress. It rejects that passage as repetitive and because Jejjata and others omitted it. Brahmadeva explained part of it, but, according to this commentator, did not examine whether it was authoritative or non-authoritative.
