Meaning of the commentary

The commentary proposes that an omitted word ‘and so forth’ should be understood, making the title ‘diagnosis of disorders from śūka and related causes.’ It takes those related causes to include intercourse involving an unhealthy vaginal condition, concealed inward-growing genital hair, intercourse with an unwilling or unsuitable partner, and trauma. Gayī rejects this expansion because those factors are already causes of upadaṃśa. Śūka is explained either as contaminated water or dirt containing small creatures, or as a preparation intended to enlarge the penis. Doṣa here may mean damage, deformity caused by incorrect use, a bodily doṣa such as vāta, or a disorder produced by such a doṣa. Since the chapter states characteristic signs rather than a separate causal account, nidāna here means diagnostic description; śūka itself is the cause and is already named in the opening proposition.

Key terms

shUka (śūka)
Explained either as contaminated creature-bearing water or dirt, or as a preparation used historically in an attempt to enlarge the penis.
shUkadoSha (śūka-doṣa)
Damage, deformity, or disorder associated with śūka or its incorrect use.
duShTayoni (duṣṭa-yoni)
A historically framed vaginal condition mentioned in one disputed expansion of the causes.
gUDhAntarlomayoni (gūḍhāntarloma-yoni)
A condition described as concealed or inward-growing genital hair.
nidAna (nidāna)
Diagnostic description in this chapter, according to the commentary.

Meaning of the commentary

The commentary says liṅgavṛddhi means making the penis larger; others specify increase in both length and circumference. Akrama means conduct outside the prescribed procedure for a penis-enlarging preparation described by Vātsyāyana and others. Some instead understand it as improper performance of an uttarabasti procedure, but Gayī rejects that interpretation. Although counting the listed diseases already yields the total, ‘eighteen’ is stated explicitly to prevent error in counting.

Key terms

li~ggavRuddhi (liṅga-vṛddhi)
Attempted enlargement of the penis, interpreted by some as increasing length and circumference.
akramapravRutta (akrama-pravṛtta)
Proceeding through an improper or non-prescribed method.
shUkadoShanimitta (śūka-doṣa-nimitta)
Occurring because of damage associated with śūka.
uttarabasti
A urethral or genitourinary instillation procedure proposed by some interpreters and rejected here by Gayī.
aShTAdasha (aṣṭādaśa)
Eighteen, explicitly stated to secure the count.

Meaning of the commentary

The main commentary explains śūkadurbhagnahetukā as caused by śūka that has been used improperly or handled badly. Others read śūkadurbhagahetukā and explain that both śūka and an unhealthy female genital condition are causes. Gayī rejects that alternative reading and explanation.

Key terms

Meaning of the commentary

The commentary connects the sentence as ‘aṣṭhīlikā arises from the śūkas.’ Viṣama means unsuitable or abnormal. It tentatively explains antaiḥ as fallen or damaged tips. Others interpret the phrase as ‘marked by irregular ends,’ meaning that the śūkas themselves have uneven tips. Viṣasambhugna means made crooked or bent by poison or a toxic substance.

Key terms

aShThIlikA (aṣṭhīlikā)
A classical hard genital lesion associated here with damaged or irregular śūka.

A classical knotted or nodular disorder associated with repeated śūka use.

Meaning of the commentary

The commentary also compares kumbhīkā with a kumbhī fruit. It interprets aśubhā by contrast with śubhā, understood as white, and therefore takes aśubhā to mean black or dark. This too is said to arise from improperly used śūka. Some read āśujā instead of aśubhā; on that reading, the lesion arises quickly after śūka is applied or inserted.

Key terms

kumbhIkA (kumbhīkā)
A classical genital lesion compared with a jambu seed or kumbhī fruit.
jAmbavAsthinibha (jāmbavāsthi-nibha)

Meaning of the commentary

The signs meant for alajī are those described for the alajī type of prameha-associated lesion. This form too is understood as following use of harmful or improperly applied śūka. Mṛdita is explained as a condition in which the penis, after śūka has been applied or inserted, is pressed very forcefully with the hands and consequently becomes irritated and swollen through vāta.
