Meaning of the commentary

Among the eleven conditions classed as chedya, the text first presents a common procedure for five varieties of arma and begins with the preparatory step. ‘After eating unctuous food’ may refer either to a person whose body has been purified and who has completed the graduated diet, or to one who has taken unctuous food according to the prescribed regimen. ‘Carefully’ means without causing excessive distress. The eye is stimulated to loosen the arma, and the saline powder is explained as one derived from saindhava.

Meaning of the śloka

Once the arma has been stimulated, quickly well fomented, and moved so that a fold forms, the physician should engage it at that site. While the patient looks toward the outer canthus, the careful physician lifts it with a hook held by mucuṇḍī forceps or with a threaded needle. It should not be raised suddenly or excessively, because of the danger of injury from the instrument, and the eyelids should be held firmly. When loosened, it is suspended securely with three supports and carefully separated with the sharp rounded-tip instrument. After freeing it throughout from the dark and white regions of the eye, it is brought near the inner canthus and excised without cutting too close to that canthus. Leaving one quarter of the tissue there is said to avoid injury to the eye; destruction of the inner canthus may produce bleeding or a sinus, while insufficient excision leads rapidly to renewed growth.

Meaning of the commentary

The commentary identifies the kind of arma that can be grasped with a hook: after it has been moved and a fold has formed, the hook is attached at that site. Mucuṇḍī is explained as forceps held between the thumb and index finger; after the hook has been secured, the arma may instead be lifted with a threaded needle. The surgeon must not lift it suddenly or too high because it may tear. The upper or lower lid is drawn and held according to the lesion’s position. ‘Loosened’ means detached from its adherence to the eyeball, and three supports rather than one are prescribed to prevent tearing. The tissue is held suspended and separated throughout with the maṇḍalāgra instrument. It is then supported by the previously placed threaded needle and cut near, but not beyond, the inner canthus. The commentary reads the retained quarter as tissue over the upper part of the eyeball rather than tissue immediately beside the canthus. Excessive excision that damages the inner canthus may cause bleeding, a sinus-like wound, and pain; incomplete excision permits the arma to grow again.

Meaning of the commentary

The commentary asks what should be done when a widely spreading arma extends over the white part near both eyelids and does not form a fold. Such a net-like, spreading growth is rubbed upward with saindhava powder to loosen it, suspended with a curved hook, and cut with the maṇḍalāgra instrument. In the expression concerning the eyelid and white region, ‘end’ is understood as proximity in relation to the eyelid and as a part or region in relation to the white of the eye. Some authorities read ‘a spreading jālaka like an arma.’ On that reading, jālaka is a sinewy arma and is suspended and cut with a curved instrument like arma, whereas an arma situated near the eyelid and white region is excised by the scraping or separating method.

Immediately afterward, the eye is treated by pratisāraṇa with powders of yāvanāla, trikaṭu, and salt. After fomentation, the skilled physician applies a bandage. Knowing the doṣa, season, strength, and stage of the condition, the physician applies a suitable unctuous substance and thereafter manages it like a wound. After three days the bandage is released, warmth is applied by the hand, and cleansing treatment is undertaken.

Meaning of the commentary

The passage sets out pratisāraṇa after excision. Yāvanāla is glossed as yavakṣāra and the salt as saindhava. Fomentation is understood as being given with the hand, and the skilled physician is one knowledgeable about bandaging and related measures. The compound concerning doṣa, season, strength, and time is explained in two ways: either as knowing the strength and timing of doṣas through their accumulation and other phases, and of seasons through their beginning, middle, and end; or as knowing the doṣas such as vāta, the seasons such as hemanta, the patient’s capacity for procedures, and the stage-specific time. A suitable unctuous substance, such as one among the ghṛta preparations appropriate to the individual, is then applied to promote union. The commentary additionally mentions a fresh-wound paste and wash prepared from substances such as triphalā, lodhra, and candana, although these are not stated in the verse. Subsequent care follows wound-management principles of cleansing and healing, with cleansing described after three days.

Meaning of the commentary

The commentary explains the procedure: nirbhujya means turning the eyelid over and making it face upward. Some transmit this reading differently, but that version is absent from the manuscripts available to the commentator and is therefore not recorded. After the operation, pratisāraṇa means rubbing the site. For remnants of arśas and the other disorders after cutting, the visual organ is covered with cloth and the remaining lesion is cauterized with a fire-heated probe; an alkaline agent may also be used for scraping. For excisable disorders whose medicine has not otherwise been specified, 'agents having both parts' means emetic and purgative substances. The commentary interprets this as cleansing the wound with their paste, decoction, or similar preparations. Some instead understand it as removing kapha and other doṣas through emesis and purgation themselves, but the Pañjikā author rejects that interpretation.
