अथातो दोषभेदविकल्पमध्यायं व्याख्यास्यामः ||१|| यथोवाच भगवान् धन्वन्तरिः ||२||

तन्त्रयुत्तयनन्तरं परिशिष्टत्वाद्दोषभेदाध्यायारम्भः- अथात इत्यादि| अथेत्यादिकं पूर्ववह्याख्येयम्| धातून् दूषयन्तीति दोषा वातादयः, तेषां भेदः पृथक्संसर्गसन्निपातभेदेन, तस्य विकल्पनमेकैकद्यनुगमनेन नानात्वकरणं प्रपञ्चनं दोषभेदविकल्पः, तमधिकृत्य कृतः, तम्||१-२||

Meaning of the commentary

The commentary says that after the tantrayuktis, the chapter on the varieties of doshas begins, as a supplement. Doshas are those that spoil the tissues: vāta and the others. Their varieties come from single, combined and all-three (sannipāta) states; 'vikalpa' is the elaboration into many by taking each in turn. The chapter is made concerning that.

अष्टाङ्गेत्यादि| अष्टाङ्गानि शल्यादीनि वाजीकरणान्तानि, तान्येव वेद आयुर्वेदः, तेन तत्र वा विद्वान् यस्तम्| महौजसं महाप्रभावम्| सूक्ष्मा लीनार्थाः , अगाधा दुरवगाहा ये आगमा एवोदधयस्ते सन्त्यस्मिन्निति| श्रीमानिति राजश्रिया ब्राह्वया वा अलङ्कृतः| ननु, विश्वामित्रो गाधिराजः, तत्सुतत्वेन राजश्रिया योगो युक्तः, कथं ब्राह्म्या श्रियेति? सत्यं, विश्वामित्रस्य ब्राह्मण्यं तपसा, ततो ब्राह्म्या श्रिया योगो युक्त एव| अन्ये तु क्षत्रियाणां ब्रह्मर्षिजातत्वेनोभययोग इति मन्यन्ते, अपरे तु विद्यासमाप्त्या ब्राह्मया श्रिया योग इति मन्यन्ते| तथा चोक्तम्- “विद्यासमाप्तौ ब्राह्मं वा सत्त्वमार्षमथापि वा| ध्रुवमाविशति ज्ञानात्तस्माद्वैद्यो द्विजः स्मृतः”- इति||३||-

Meaning of the commentary

The commentary explains the epithets. The eight limbs are surgery (śalya) and the others, ending with aphrodisiac therapy (vājīkaraṇa), and the Veda is Āyurveda itself; he is learned in it. 'Of great strength' means of great power. The 'ocean' is subtle, hidden and hard-to-fathom scriptures. 'Glorious' means adorned with royal glory or the glory of a Brahmin. An objection: Viśvāmitra was King Gādhi's son, so royal glory fits his son, but why Brahmin glory? The answer: Viśvāmitra's Brahminhood was earned by austerity, so Brahmin glory suits. Others say that because kṣatriyas became sons of Brahmin sages, both apply; others say Brahmin glory arises by completing learning. A quotation is given: at the completion of learning a Brahmin or sage-like purity of mind surely enters through knowledge, therefore the physician is called 'twice-born'.

Meaning of the commentary

The commentary says 'declared earlier' refers to the chapter on the analysis of tastes. Why sixty-two dosha varieties and sixty-three taste varieties? It answers with a quoted verse: sixty-two are the dosha varieties, sixty-three the taste varieties, and health is the sixty-third, to be known, and there the six tastes are applied. Asked how many are known singly, in pairs or in threes, it quotes: six singly, twenty-one with four added (in pairs), thirty-two in threes; thus the three doshas in sixty-three ways.

Meaning of the commentary

The commentary says that having heard the pupil's question, the teacher replies. 'His' means Suśruta's; 'that' means what was said earlier. 'Remover of doubts' means cutter of uncertainty; 'pleased at heart' means with satisfied mind; 'tiger among kings' means best of kings; 'truth' means as it really is.

Meaning of the commentary

The commentary says the three doshas are vāta and the others, and the tissues are the seven beginning with rasa; 'unaffected' means not gone into abnormality; 'wholesome tastes' means sweet and the other tastes that are wholesome. An objection: since the question asked how many are singly known, why start with 'the three doshas'? The reply: to show that when tissues and wastes meet the doshas, they take the name dosha; hence earlier fifteen spreads were described with blood. Why then are spreads with flesh and others not stated? Because they are countless, and one knows them by the signs of the doshas. As the text will say: 'therefore restraining the extension'. Another objection: 'body has dosha, tissue and waste as its root' already says the body arises from them, and 'body' and 'deha' are synonyms, so why say 'sustain the body'? Answer: here 'deha' means the person of action, who is the subject of treatment.

Meaning of the commentary

The commentary asks what kind of person 'deha' means, and answers with the person of action already described, and, by extension, gives other counts to aid memory. On 'sixteen kalās' some take it as the five elements, eleven senses, making sixteen modifications; the commentary says this is not accepted as authoritative in Āyurveda. Others take 'kalā' to mean limbs and sub-limbs: head, neck, hands, feet, sides, back, abdomen and shoulders as eight limbs; chin, nose, lips, groin, thumb, fingers, heel and ankles as sub-limbs. Some teachers of independent and dependent views read 'kalā' as quality (guṇa), meaning sixteen qualities described in the chapter on consideration of all beings and body: pleasure, pain, desire, aversion, effort and the like; Caraka says the four-footed treatment has sixteen kalās, explained as sixteen qualities. The eleven prāṇas are: fire, soma, vāyu, sattva, rajas, tamas, the five senses and the bhūtātman; the bhūtātman alone is the sixteen-kalā one, and only that which is joined is called prāṇa, hence eleven. The one thousand one hundred and twenty disorders (eleven hundred and twenty) are then listed in a long series of verses attributed to the Suśruta text by section: in Sūtrasthāna sixty-six disorders (burns, wounds by fire, alkali, blood-related and others); in Nidānasthāna (the six sections) disorders of vāta, urinary, skin, abdominal, wounds, fistulae, eye, ear, nose, mouth, gynaecological and other categories; in Cikitsāsthāna 98; in Kalpasthāna disorders caused by poison from stationary and moving creatures with counts of bite phases and 247; in the Uttaratantra eye (76), ear (28), nose (31), head (11) and other disorders totalling 146; children's disorders (nine bālagrahas and twenty gynaecological), fevers, diarrhoeas, dysenteries, phthisis, gulma, heart diseases, jaundice, bleeding disorders, fainting, alcohol disorders, burning, thirst, vomiting, hiccups, breathlessness, cough, voice loss, worms, obstructions, urinary obstructions (147 in the general medicine part); afflictions from gods, demons and others (eight), apasmāra (four) and unmāda (six), totalling eighteen in the Bhūtavidyā section; thus counted by Kāśīrāja for the Suśruta text. The verse stating 'one thousand one hundred and twenty disorders' is explained as 'in brief' with 'in detail' distinct, and 'hi' as giving the reason.

For example, vāta is predominantly rajas, since rajas is the mover of all states; pitta is sattva-rich because of lightness and illumination, or, some say, joined with rajas; kapha is rich in tamas, being heavy and covering, as the physicians say. An objection: then how is a kapha-constitution person said to have sattva quality? The reply: both other qualities are also to be understood in kapha, since it is said 'water is rich in sattva and tamas'.

Meaning of the commentary

The commentary explains how the sixty-two arise. Singly: one dosha aggravated and the other two normal (vāta alone, pitta alone, kapha alone: 3). Pairs: with two equally increased and the third normal (3); with one of the two more increased (6); total nine. All three: thirteen (all three equal: 1; one dominant: 3; two dominant: 3; graded low-middle-high in six orders: 6). So the aggravated varieties total 25 (3+9+13). Likewise for declined doshas: 3 singly, 9 in pairs, 13 all-three, totalling 25; 25 + 25 = 50. Then the mixed states of decline, normal and increase (six with one declined, one normal, one increased; three with two declined and one increased; three with two increased and one declined) add 12, giving 62, with the balanced (all-normal) state the sixty-third, health. The commentary quotes a long series of verses stating the same enumeration, and it lists signs: in vāta increase, much talk, thinness, dusky colour, twitching, love of warmth, loss of sleep, weakness and hard stool; in pitta increase, yellowish appearance, heat, desire for cold, little sleep, fainting, loss of strength, weak senses and yellow stool, urine and eyes; in kapha increase, whiteness, coldness, steadiness, heaviness, lassitude, drowsiness, sleep and joint and bone looseness; in vāta decrease, slow movement, little speech, lack of joy, confusion; in pitta decrease, poor warmth and digestion, lack of lustre; in kapha decrease, dryness, internal burning, emptiness of the stomach and head areas, loose joints, thirst and weakness. It also quotes verses on combined states, such as one dosha increased, one balanced and one decreased, and their effects (burning, pain, cold, heaviness, fever, headache, loss of digestion, tremor, stiffness and others), ending that a physician should treat by keeping the balanced dosha safe, reducing the increased and increasing the decreased.

Meaning of the śloka

Doshas mixed with the tissues and wastes go again into countless varieties. Therefore, restraining extension and diagnosing the disease by the analysis of dosha varieties, one should treat with the taste varieties as stated. The physician is the doer, the taste-substances are the instrument (karaṇa), and the doshas are the cause; the effect is only health, and its opposite is non-health.

Meaning of the commentary

The commentary says doshas mixed with tissues such as blood, and with wastes, become countless. The signs of vāta mixed with tissues and wastes are stated in the chapter on vāta disorders. For pitta and kapha, Vṛddhavāgbhaṭa is quoted: pitta in the skin causes boils and smallpox-like eruptions; in blood, spreading eruptions and burning; in flesh, rotting of flesh; in fat, burning nodules, sweating, thirst and vomiting; in bone, bone burning; in marrow, yellow nails and eyes; in semen, foul yellow-looking semen; in vessels, anger; in sinews, thirst; in the abdomen, intoxication, thirst, burning; and other pervasive effects. Kapha in the skin causes stiffness and whitish look; in blood, pallor; in flesh, tumours and cervical nodules; in fat, stoutness and urinary disorders; in bone, stiffness; in marrow, whitish eyes; in semen, accumulation; in vessels, obstruction and heaviness; in sinews, joint swelling; in the abdomen, distension of the belly, anorexia and indigestion. Waste-related effects are also listed. The verse ends with four points: the doer is the physician, the instrument is the taste-substance, the cause is the doshas, and the only effect is health.

Meaning of the śloka

Of the sixty-six chapters, this treatise, with arrangement of meaning, words and sequence, is thus complete in the Uttara (last part), prosperous. For those of dull, shallow minds who cannot grasp what is clear and hidden, it has been explained in due order and according to the questions. Having studied all this with the Uttara, with the Brāhma rite as stated, one does not fall short of the goals of the mind; this word of Brahmā is truly very true.
