Meaning of the commentary

After describing medicinal treatment for disorders among the four kinds of abhiṣyanda following raktābhiṣyanda, the text now begins to explain disorders treated by operative measures. Because lekhya-añjana has just been discussed, it first presents the treatment of diseases indicated for lekhana. Although the earlier classification, beginning with the statement that eleven conditions are chedya, could suggest that chedya disorders should be taken first, lekhya disorders are presented beforehand because of that immediate context and because lekhana may also be used in the mild stage of disorders otherwise treated by chedana and related procedures.

Meaning of the śloka

A common procedure is stated for the nine disorders described as lekhya. After oleation, emesis, and purgation, the person is placed lying face upward in a sheltered, sun-warmed room and held securely by trusted attendants. The eyelid is everted and gently warmed with cloth heated in comfortably warm water. With the finger and thumb covered by cloth, the everted lid is held steadily so that it neither moves nor slips. After wiping it with cloth and marking the operative site, the lid is scraped with an instrument or with leaves. When bleeding has ceased, the warmed site is rubbed with finely powdered manohvā, kāsīsa, vyoṣa, ārdrāñjana, saindhava, and mākṣīka mixed with honey. It is then washed with warm water, covered with ghee, and cared for like a wound. After three days, measures beginning with fomentation and avapīḍa are prescribed. Meaning of the commentary

The commentator explains that the verse first counts the disorders and then gives the procedure common to all nine. Fomentation, though not expressly stated, is understood after oleation. The warming should be controlled so that the eyes are not distressed. Nirbhujya and nirbhugna mean turning or everting the lid, and the cloth-covered finger and thumb keep it from trembling, moving, slipping, or falling downward. The phrase concerning the instrument-marked site includes a site that has been punctured where puncture is indicated, opened where opening is indicated, or excised where excision is indicated. A hard, severe eyelid disorder dominated by vāta and kapha is said to be scraped with an instrument, whereas a soft lid affected by pitta and rakta is scraped with leaves such as those of śephālikā. For aftercare, manohvā is identified as manaḥśilā, ārdrāñjana as rasāñjana, mākṣīka as suvarṇamākṣīka, pratisārya as rubbing, and havis as ghee. The site is treated like a fresh wound for seven days and thereafter like an ordinary bodily wound, including measures such as a leaf bandage. After the third day, fomentation, avapīḍa, and related measures are again used as wound care. The final statement refers these briefly named measures to their fuller descriptions and applies the regimen to all disorders considered suitable for lekhana.
