Meaning of the commentary

The commentary defines upadrava as a disorder that troubles the patient and says that this chapter mainly concerns complications arising from pancakarma. Their treatment is described as measures opposed to the cause and qualities of the complication. It preserves an alternative chapter title, Aturopakramacikitsita. It also explains that procedures from oleation through niruha may temporarily weaken agni; therefore the chapter teaches gradual diet beginning with peya to restore it, and discusses disorders produced when prohibited actions such as anger are undertaken through prajnaparadha.

Key terms

Aturopadrava (aturōpadrava)
A complication or additional disorder affecting a patient.
pa~jcakarma (pañcakarma)
The group of procedures identified as a frequent cause of the complications discussed here.
kAyAgni (kāyāgni)
Digestive capacity, said by the commentary to become temporarily weak after the procedures.
praj~jAparAdha (prajñāparādha)
An error of judgment associated here with undertaking prohibited conduct.

Meaning of the śloka

After oleation, emesis, purgation, bloodletting, niruha-basti, or anuvasana-basti, a person's digestive fire becomes weak. Just as a small fire is extinguished by large or excessive fuel, heavy food taken in excess suppresses this weakened fire. Small quantities of light food gradually increase it, as a fire is kindled by small, fine pieces of wood.

Meaning of the commentary

The commentary presents this as a reasoned precaution intended to prevent complications after procedures that begin with drinking sneha and extend through the five cleansing procedures. The conjunction in the verse is said to include a person who has received anuvasana even though that procedure is not explicitly named. It addresses an apparent contradiction: elsewhere these procedures are said to awaken agni, but here agni is weak. The answer is that the weakness occurs immediately, whereas agni becomes stronger gradually. The following dietary sequence is therefore intended to restore the person's usual state step by step.

Key terms

kAyAgni (kāyāgni)
Digestive capacity, described as temporarily weakened immediately after the procedures.
nirUDha (nirūḍha)
A person who has undergone niruha-basti.
anuvAsita (anuvāsita)
A person who has received anuvasana-basti, included by the commentary.
laghvanna
Light food used in small amounts in the gradual dietary restoration.

Meaning of the commentary

The commentary explains three graded sequences corresponding to the three measured degrees of elimination. It equates yavagu here with peya and interprets the singular, dual, and plural forms as one, two, or three meal-times. It then defines vilepi as overcooked rice made from one quarter of the person's accustomed rice amount and without manda; the next rice is made from one half and accompanied by unseasoned mung soup; the next uses three quarters and a seasoned soup; and the final stage restores the full accustomed quantity with prepared meat broth. For low elimination it lays out one meal-time at each stage before return to the usual diet; for middle elimination each stage lasts two meal-times; and for highest elimination each lasts three. The commentary rejects a line found in some manuscripts because Jejjata and other commentators omit it. It also records Gayi's somewhat different account of the low sequence and cites another treatise summarizing the graded meal-times. These are separate historical explanations and should not be combined into a single modern regimen.

Key terms

peyA (pēyā)
Thin rice gruel used at the beginning of the graded sequence.
vilepI (vilēpī)
A thicker rice preparation described by the commentary as overcooked and without its liquid portion.
akRutamudgayUSha (akṛta-mudgayūṣa)
Clear mung soup without the oil and salt used for seasoning.
kRutayUSha (kṛta-yūṣa)
Seasoned soup used at a later stage of the sequence.
mAMsarasa (māṃsarasa)
Animal broth used in the final stage described by the source.

Meaning of the commentary

The commentary fixes the preceding rule: when one prastha has been eliminated, classified as low purgation, each stage beginning with peya is given for one meal-time; with half an adhaka, the middle degree, each is given for two meal-times; and with one adhaka, the highest degree, each is given for three. It summarizes the total period as two days for the low degree, four for the middle, and six for the high, assuming morning and evening meals. The sequence is called proper because it is adjusted to the stated amount of dosa removed rather than being insufficient or excessive.

Key terms

hInavireka (hīna-virēka)
The lowest degree of purgation in the commentary's threefold measurement.
madhyamavireka (madhyama-virēka)
The middle degree of purgation.
uttamavireka (uttama-virēka)
The highest degree of purgation.
annakAla (annakāla)
A meal-time used to count the stages of the dietary sequence.

Meaning of the commentary

The commentary presents an exception to the peya sequence. In people with predominant kapha and pitta who habitually consume alcohol and have undergone insufficient cleansing, it says that peya may obstruct or congest the channels. It therefore recommends beginning historically with tarpana made from parched-rice flour, followed by soup, rice, and the later stages. A cited passage from another treatise also advises against peya in a person with little pitta and kapha eliminated, in a habitual drinker, or in a vata-pitta condition. To answer the objection that this makes the earlier peya rule for low cleansing useless, the commentary limits the exception to the kapha-pitta-predominant habitual drinker and applies the earlier rule to other cases.

Key terms

madyanitya (madya-nitya)
A person habitually consuming alcoholic drink.
hInavishodhita (hīna-viśōdhita)

abhiShyanda (abhiṣyanda)
Congestion or obstruction attributed here to peya in the specified group.
tarpaNa (tarpaṇa)

Meaning of the commentary

The commentary applies, by extension, the graduated dietary restoration described after cleansing to some people whose digestive fire is weakened even though they have not undergone cleansing. It preserves two explanations of vedanālābha: some understand it as failure to obtain something desired, while others understand it as obtaining something unwanted. Niyama includes fasting and similar observances, and vaicittya includes conditions such as unmāda. For people deprived of food for these reasons, dietary restoration is said to last two, four, or six days according to the lower, middle, or higher degree of fasting. These periods describe the historical regimen and are not present dietary instructions.

Key terms

vedanAlAbha (vedanālābha)
An ambiguous expression explained either as not obtaining what is desired or as obtaining what is unwanted.
niyama
A restrictive observance, including fasting in the commentary.
vaicittya
Mental disturbance, exemplified by unmāda in the commentary.
viriktavat
In the manner of caring for a person after purgation.
annasaMsarjana (annasaṃsarjana)
The graduated restoration of food described after fasting or cleansing.

Meaning of the śloka

The measures expressed as an āḍhaka, half an āḍhaka, and a prastha are associated here with purgation. Those knowledgeable about purgation do not accept those measures as decisive, because a proper purgation ends with the passage of kapha. The text therefore recognizes one principal purgation ending in kapha, not a second kind.

Meaning of the commentary

The commentary first presents a view that fixed measures such as an āḍhaka or prastha belong specifically to purgation because purgation can greatly weaken digestion, whereas emesis and āsthāpana do not overwhelm the seat of digestive fire in the same way. It then rejects that position. Purgation is intended to remove pitta completely, and this is inferred when kapha in an āma-like state from another location appears after pitta; specialists therefore do not treat volume as the determining standard. This reasoning also rejects counting bouts as decisive, while assessment by signs is regarded as fully valid because it too includes ending in kapha. The commentary cites an order of materials passed in emesis and purgation. Count- and volume-based assessments are treated as secondary accommodations that may sometimes be applied in children, older adults, or other particular conditions; the primary type is the single purgation recognized by its kapha ending. It says the same principle applies to emesis and quotes Bhoja in support. These are historical criteria, not current procedural guidance.

Key terms

ADhaka (āḍhaka)
A historical volume measure discussed as an assessment of purgation output.
prastha
A historical volume measure whose decisiveness is rejected in the principal interpretation.
shleShmAnta (śleṣmānta)
Ending with kapha, treated by the commentary as the principal sign of completed purgation.
vaigika
Assessment based on the number of evacuations, regarded here as secondary.
lai~ggika (laiṅgika)
Assessment based on clinical signs, accepted by the commentary as the valid standard.

Meaning of the śloka

Because strength is described in three degrees, the dietary sequence is also threefold. A person of strong bala should follow one sequence once, one of moderate bala twice, and one of weak bala three times. Some authorities instead apply this same gradation to weak, moderate, and strong digestive fire.

Meaning of the commentary

The commentary explains this threefold dietary restoration by reference to bala because purgation itself was just described as fundamentally one rather than three kinds. Here bala means bodily development or nourishment. Although all three forms of dietary restoration might theoretically apply across all three degrees of strength, the verse assigns a particular repetition to each. The closing statement preserves another opinion: some authorities correlate the threefold sequence with weak, moderate, and strong digestive fire rather than with bodily strength.

Key terms

bala
Strength understood here by the commentary as bodily development or nourishment.
anukrama
The ordered dietary restoration sequence.
durbala
A person classified as weak in the verse.
agni
Digestive fire, classified by some authorities as weak, moderate, or strong.

Meaning of the commentary

The commentary explains the sequence after completion of dietary restoration. Unctuous tastes are grouped as sweet, sour, and salty, while dry tastes are grouped as pungent, bitter, and astringent, and they are used in alternating order. Sweet and bitter are placed first to counter the vāta and pitta associated with increased digestive fire and to steady agni. Unctuous, sour, salty, and pungent tastes then oppose vāta and kapha and kindle agni. Sweet and bitter are repeated to counter pitta and vāta said to arise from sour, salty, and pungent tastes, followed by astringent and pungent. Alternating mutually opposed unctuous and dry tastes is said to restore the person's usual state. A verse attributed to Vṛddha Vāgbhaṭa gives a comparable sequence. The source text and commentary show some compression in the ordering, so the detailed sequence should be checked against the Sanskrit rather than treated as a menu.

Key terms

saMsarga (saṃsarga)
The graduated dietary restoration after cleansing.
agni
Digestive fire, described as having increased through dietary restoration.
rasa
Taste categories introduced in the order discussed by the text.
snigdha
Unctuous, applied here to the sweet, sour, and salty group.
rUkSha (rūkṣa)
Dry, applied here to pungent, bitter, and astringent tastes.
vyatyAsa (vyatyāsa)
Alternation or reversal in the use of opposed tastes.

Meaning of the commentary

The commentary clarifies that the full graduated dietary rule stated for oleation and emesis as components of pañcakarma does not apply in the same way to other circumstances. 'Only having consumed sneha' refers here to taking an unctuous substance to pacify a doṣa or disorder. 'Only having vomited' refers to vomiting induced because of undigested food or a similar cause. Light food means light both in quantity and in the nature of the food. After this period, the person is said to introduce sweet and the other tastes according to the sequence stated immediately before.

Key terms

snehapIta (snehapīta)
A person who has consumed an unctuous substance, here outside the full cleansing sequence.
vAnta (vānta)

Meaning of the commentary

Among the previously mentioned groups, the commentary gives special attention to the recovery period for a person who has undergone venesection and one who has undergone purgative cleansing. After seven nights of graduated dietary restoration and subsequent use of tastes in the stated order and alternation, rasa is said gradually to nourish successive dhātus, with strength returning over about a month as the sequence reaches śukra. The person is therefore told to avoid the conduct beginning with anger that is listed afterward. Recovery does not occur at the same speed in everyone: one whose digestive fire is extremely weak may regain strength only after a month, whereas one with strong digestive fire may do so sooner. The commentary uses analogies of the spread of water and sound to describe these different rates. This is the source's historical account of recovery, not a prediction of present clinical healing.

Key terms

sirAvyadha (sirāvyadha)
Venesection or puncturing a vein, one of the procedures named in the verse.
shodhana (śodhana)
Cleansing, specified by the commentary here as purgation.
parihAra (parihāra)
The restrictions or precautions to be observed during recovery.
rasa
The nutritive substance said in the commentary to pass gradually through successive tissues.
dhAtu (dhātu)
A bodily tissue in the commentary's account of gradual nourishment and recovery.
bala
Strength whose return determines the duration of restrictions.

Meaning of the commentary

The commentary distinguishes basti from the fixed dietary transition prescribed after the other cleansing or oleation procedures, because basti may be repeated according to the strength of the doṣa and disorder. Even so, it assigns a specific three-day period of restrictions after each basti. It interprets the next stage as resuming what is suitable from about the fourth or fifth day, without exceeding the signs of properly completed nirūha or anuvāsana. Another interpretation applies the three-day restriction specifically after each anuvāsana and, after the third anuvāsana, allows a further anuvāsana after an interval of four or five days, according to the patient's condition and agni. These are distinct historical readings, not a current treatment schedule.

Key terms

parihāra
The prescribed period of restrictions following a basti.
basti
The invasive rectal procedure discussed in this passage.
nirūha
A type of basti whose proper completion signs are considered by the commentator.
anuvāsana
An unctuous type of basti to which the alternative interpretation applies.
agni
Digestive capacity considered in the alternative account.

Meaning of the śloka

The verses attribute numerous complications during convalescence to disregarding prescribed restraints. Anger is said to aggravate pitta and produce complications, while exertion or grief may disturb the mind. Sexual activity is linked with severe disorders including convulsions, paralysis, stiffness, genital swelling, severe cough and difficult breathing, and discharge of blood resembling semen or mixed with menstrual blood. Daytime sleep is said to produce kapha-related conditions such as splenic enlargement, abdominal disease, nasal catarrh, pallor, swelling, fever, confusion, bodily weakness, indigestion, loss of appetite, heaviness, and excessive sleep. Loud speaking is linked to head pain and disorders of sight, cognition, smell, hearing, speech, jaw movement, eyes, thirst, cough, wakefulness, and loose teeth. Travel by animal or chariot is associated with vomiting, fainting, dizziness, fatigue, stiffness, and sensory disturbance. Prolonged sitting or standing is linked with pain in the hips; excessive walking with pain or wasting of the legs, swelling, or abnormal sensation in the feet. Exposure to cold things, pleasures, or water is said to increase vāta and lead to body ache, obstruction, colic, distension, and trembling. Wind and sun are respectively linked with altered complexion and fever. Incompatible food or eating again before digestion is complete is said to cause grave disease or death, while unsuitable or excessive food diminishes strength and complexion and produces indigestion, described as the root of many disorders.

Meaning of the commentary

The commentary presents these as consequences of failing to observe the restrictions prescribed for five previously discussed patient groups. It glosses mental disturbance broadly, including confusion, intoxication-like disturbance, apasmāra, and unmāda; explains loss of smell, muteness, and adhimantha; and specifies travel on animals or in a chariot. It interprets prolonged sitting and standing separately and defines excessive walking as roaming about. For the leg symptom, it records the variant reading svāpa, numbness, in place of śopha, swelling. It explains sambhoga here as enjoyment of items such as garlands and sandalwood, while Gayī instead reads 'cold food and water.' Wind is assigned altered complexion and sun-aggravated pitta is assigned fever. Incompatible eating is illustrated by combinations such as milk and fish, while adhyāśana means eating while the previous food remains undigested. Indigestion is divided into āma, vidagdha, and viṣṭabdha forms associated with the doṣas, with rasājīrṇa counted as a fourth. The commentary counts fifteen categories of mishap across the listed causes. These associations belong to the historical explanatory framework and do not establish modern causation.

Meaning of the commentary

The commentary summarizes treatment by distinguishing a remote cause, such as anger, from an immediate cause, such as pitta. The opposing measure may be directed against the remote cause, against the complication itself, or against the implicated doṣa such as vāta. The reference to a discerning physician emphasizes reasoned assessment in seeking the patient's recovery. This is a general historical therapeutic principle rather than a specific modern intervention.

Key terms

vyāpad
A complication or adverse condition.
viprakṛṣṭa kāraṇa
A more remote cause, illustrated by anger.
sannikṛṣṭa kāraṇa
An immediate cause, illustrated by pitta.
pratyanīka
A measure understood as opposing the identified cause, complication, or doṣa.

Meaning of the commentary

After stating that unsuitable food reduces strength and complexion, the commentary identifies what the passage considers suitable. Beyond the expressly named items, it includes other foods described as light within the categories of viṣkira birds, animals of arid terrain, rice and ṣaṣṭika grains, and pulses such as mudga. Jejjaṭa gives another explanation: the passage extends to a person after emesis the regulated dietary progression already prescribed after purgation, because that progression, repeated once, twice, or three times according to the degree of cleansing, had been stated for purgation but not explicitly for emesis. The two explanations are preserved separately. The animal foods and dietary sequence are historical descriptions, not individualized dietary instructions.

Key terms

virikta
A person who has undergone purgation.
vānta
A person who has undergone therapeutic emesis.
ṣaṣṭika
A quickly maturing rice variety listed as a light food.
purāṇaśāli
Aged śāli rice.
mudga
Green gram, included among the light foods.
laghu
Light, describing food considered easy in the source's dietary framework.
